1. Prepare for your ap- pointment ahead of time: write down questions you want to ask and topics that you'd like to discuss, and ask what test results or other materials will be necessary for you to have on hand.
2. When possible, bring a friend or family member along to your appoint- ments. That person can keep notes and help you remember important points. (Recording discus- sions with your care team can also be helpful.)
3. Be open and honest with your provider about your questions and con- cerns to ensure an effective partnership.
